Came home from work today and noticed my Achilles tang had one cloudy eye. No other marks at all on him, and swimming and acting completely normal. He has been in the tank for 2 years no issue. I QT all fish that have entered the system, but no new fish or corals in over a month.

any ideas what could be going on?

When just one eye is involved, it is almost always an injury. Because Achilles don’t like to be moved into small tanks, I wonder if you might be better off just watching it for now? The common treatment is erythromycin in a hospital tank, but the reality is these fish may just get better or not, regardless of treatment.
