At a loss on Royal Gramma scratching

I have been treating my tank with Paraguard for 30 days for what appeared to be ich on my royal gramma.
He started to scratch himself on the sand 3 or 4 days ago. So I gave him a 4 minute dip. He is still scratching.
Any advice what the problem might be and how to treat? Thank you!

Can you post a pic of the fish?

Usually a freshwater dip doesn't start to drop off flukes until the 3 minute mark. Usually do 5 minutes. Did you use a dark colored container to look for the fluke bodies?

He looks and acts fine. Has been eating too. I hate to subject him to another dip. I did the 4 minute one two days ago.
