Looks like an injury and likely a sting from a rabbitfish or even puncture from another tang. I was just able to expand the pic and its literally a hole. I cant imagine anything in form of live creature has done this as this is a first.
Tough to see. Could be an injury. Any other tangs in the tank? How long have you had this fish?
Jay
I do have a rabbit fishLooks like an injury and likely a sting from a rabbitfish or even puncture from another tang. I was just able to expand the pic and its literally a hole. I cant imagine anything in form of live creature has done this as this is a first.
Im imagining a 5 min freshwater dip would assure nothing within hole but working with specimen scrape and other internal, Im putting you back to @Jay Hemdal to do an assessment
I have a yellow tang, foxface, blue hippo tang, and bristletooth tang. I've had this naso tang for 2 years
Without knowing that, it is the only fish I know that would make a puncture of this nature. With that- I gave you over to Jay although I have a clue to whats next but in the mean time, you can setup a quarantine tanks as you will need it. IF you have Ruby Rally Pro on hand , you can give it a 60-90 minute Bath.I do have a rabbit fish

It can be as simple as coming too close, not necessarily attack. Jay does work and may have gone to sleep . If you have ruby rally pro, give it the 60-90 minute bath previously mentioned then return to tank and continue with rally pro treatment.Darn, he's usually so chill
Don't have rally pro. I'll check lfs tomorrowIt can be as simple as coming too close, not necessarily attack. Jay does work and may have gone to sleep . If you have ruby rally pro, give it the 60-90 minute bath previously mentioned then return to tank and continue with rally pro treatment.
SHOULD it become infected, add fish to QT and treat with Seachem Kanaplex for at least 5 days and utilize added aeration with air stone
If fish is eating and breathing normal, you are ok. If the hole appears inflamed, a 5 minute freshwater dip in water the same temperature as display tank will buy a little timeDon't have rally pro. I'll check lfs tomorrow
