Blue Spotted Puffer Fish-skin infection, parasite??

Hi all, I need your help!
We’ve had this blue spotted puffer for 1 month. None of the other fish in the tank show any sign of illness. It’s a fish only tank. He’s still eating and rests on rock throughout the day. The spots started off dark, then faded to white on one side and this morning I noticed the white bump. Can you help me figure out what’s wrong with him?

tank mates:
-2 clowns
-1 damsel
-turbo snails
-scarlet hermits

water parameters: (API)
Ph-8.0
Salinity-1.027 (usually 1.025)
Alkalinity-7
Ammonia-0.25
Nitrites-0
Nitrates-20

Unusual although not impossible for rest of them not to contract disease if it’s ich
What symptoms are you seeing such as heavy breathing, loss of appetite, etc

No symptoms. Acting like he normally does (no heavy breathing and has normal appetite)

ee ich as suspected and do see lymphocystis which is a viral condition and not disease
It is often associated with poor water quality and/or disease.
There are a couple dots on the body which may suggest bacterial or even a worm
Ruby rally pro would treat both, is mild for the puffer and safe to use

Thank you, so much! I will treat for both and do a 20% water change. If i manually top off instead of auto, will it be as effective if i create a schedule? Anything else I should do? I read that lymphocystis isn’t treatable. What are your thoughts on this?

Lymphocystis isn’t treatable, but also is hardly ever fatal, it goes away in 6 to 8 weeks

Not salinity swings. Focus on ammonia and nitrate. Its lympho.
What test kits are you using and also what foods are you feeding?

Using API and seachem. Doing a water change lowered our nitrate to 10. Feeding brine shrimp and clams.

Update: today, he is flashing and not eating. Appears to be hiding rather than resting on his normal rock.

Flashing a sign of flukes as is heavy breathing , yawning, twitching head or body and loss of appetite due to flukes in gills.
You can as long as fish is not breathing heavy give it a 5 min freshwater dip in a CLEAN bucket the same temperature as display tank which will offer temporary relief and even dislodge some of the flukes. Treatment in a separate setting will be prazi pro for 2 eight day intervals at 90% of recommended dosage.
Increase aeration with an air stone as Prazi does reduce both oxygen and appetite.
Other foods to offer when fish comes around is :
Spirulina brine shrimp
Mysis shrimp
Small plankton
LRS fish frenzy

Update: 6 flukes came off of him during the freshwater dip! Yuck!
Those would be Neobenedenia - they are egg layers and tough to get rid of. Prazipro, dosed 3x, 7 to 9 days apart is one option to try.

Ons should be sufficient. Jay chimed in- was cutting grass ( Have Lots of it)
