Blue Tang Died, Clown Head Down

Hi Reefers,

Problem
Today is a very sad day for me, my Blue Tang died this morning and I'm not able to find an explanation.
Moreover my Clown Fish is swimming head down. See Video attached + on Youtube (https://www.youtube.com/shorts/CeZZ73z4gFw)
The Clown fish is not eating anymore and have a high 'breathing' frequency.
Also, the Clown fish has a white substance on the eye.

Context
I'm a beginner, I set up a nano tank (10 gallons) 5 months ago. I did reading before starting the setup.
I don't use a skimmer, but I do weekly cleaning and weekly 30% water change.
I had a blue tang, a clown fish, a shrimp, and a sea slug.

Recent change to my tank
- Two weeks ago:
I did an extensive cleaning (mostly the sand) by removing both fish from the tank. The Blue Tang got very very stressed, he jumped on the floor but I was quickly able to put it back in the water. He became very afraid of me, every time I approach the tank, he hides.
Same day, I added the shrimp, and the sea slug without quarantine.
I also upgraded to a new light (I previously used a white light, I have now an all day automatic light providing white, red, green, and blue colors).
I also added a wave maker.

Two days ago:
- I did another cleaning and I added more bacteria (I don't know why I did that to be honest, I cleaned the filter with the aquarium water and I felt like I need to add more bacteria). Since adding the bacteria, the water became cloudy.

Aquarium Parameters:
Aquarium type: Fish-only
Aquarium water volume: 10 gallons
Filtration type: kind of sponge, not sure what's the name
Lighting: New lighting with different color options.
How long has the aquarium been established: 5 months

Blue tang should be in at least a 6' long tank, preferably 180g. Most likely stress induced illness. Can't see anything wrong with the clown. The tank is very cloudy though. Is that sand blowing or bacterial bloom? Might need to increase surface agitation.

Definitely bacteria bloom. I recently added bacteria.

Thank you for your response. I also think it's stress. My mistake for keeping it on a small tank.

Dd the sea slug die? Sometimes when they do, they release toxins into the water.
What are you testing ammonia with? Your water is cloudy/foamy and I worry that your ammonia level might be high.
Finally, the clown is breathing really fast. That is a symptom of water quality issues or gill disease..

Sea slug is doing well.
Water is just recently cloudy because I introduced new bacteria. Might be a bacteria bloom.

More surface agitation while waiting on test results may help.
