Clownfish suddenly became sick

Hi all,
Can someone please help me and tell me what is wrong with this clownfish and how to treat it? It was perfectly fine a couple of hours ago when I fed the tank. A little later I found it hovering just above the sand in the back corner of the tank. Now, it alternates between swimming nose up and nose down in the top corner. I can't see any other visible signs of disease on it and the other fish in the tank seem fine. I should add that this fish has been in the main tank since since April 20 of this year with no problems. It has gone through a quarantine of 2 weeks in a full concentration of Copper Power, then 5 treatments every other day of MetroPlex, followed by 2 treatments of praziPro...spaced 1 week apart, and 10 days of food treated with MetroPlex bound by Focus. I can’t view the video but if it was fine and ate a couple hours ago… and is just now swimming funny, it could just be sleeping. Is it lights out or blues only during this behavior? My clown has gone through phases where he used to just hang out at the back of my tank swimming nose down and hosting the temperature probes. Now he sleeps in the top corner of the tank and kind of floats on his side at the surface until it’s time to get up. Hopefully someone else that can view your video has some other insight.

Fish is somewhat thin and may be either a bouyancy issue or weakness.
is it currently eating ?
What foods of so are you offering?
How is its' breathing rate (labored or normal ) ?

He's just never acted like this before.
Temp - 77.6° F
Salinity - 1.025
Ammonia - 0 ppm
Nitrate - 0 ppm
Nitrate - 3 ppm
Phosphate - between 0 and 0.25 ppm (cheap API kit)
Ph - 8.0

It eats like a pig. I feed PE mysis, brine shrimp, Reef Frenzy, spirulina brine shrimp, and New Life Spectrum pellets.
