Coral beauty swimming weird and died

Last night our coral beauty started swimming erratically in the qt tank, then swam straight up almost like to jump out of the tank, hit the lid and was swimming almost stunned after that. After a bit she went back to normal, but we woke up this morning and she was dead.
The rest of the qt fish are acting and eating fine. So What could have caused this? Will it effect any of the other fish in qt?

Could have been bad flukes maybe.

Treating for preventative before putting in DT, Using cupramine, and prazi pro. Had it going on about a week now. Looked to be doing perfectly fine, and now this

this morning found our coral beauty dead on the floor. We think the powder blue tang and pruple tang ganged up to chase him out of the tank but we were asleep so didn't witness the crime/incident. sucks
