What type of coral is that?
Card to tell what it's supposed to look like if we don't know what it is.
It looks as though it's an LPS?

Is it getting a lot of flow? Recently added to the tank? Any parameters?

Gonna need alot more info to help you out much.

It's a donut or meat coral. Getting the same flow as always Params are as follows..SG 1.024, Alk 8.7, Phos .07, Nitrates 8.0 Have had it a while but I have been battling cyano on my sand bed for some time. Considering carbon dosing?

Any swings in parameters lately? Temp, pH. Alk, or Salinity in particular. Often bailout is caused by swings (its basically a corals last ditch effort at survival when conditions aren't to their liking. They release from the skeleton and hope to drift to better conditions)

It's clearly bothered by something. That's what causes polyp bail. So we can explore other options. Any fish or inverts that could be irritating it? I know I've had cleaner shrimp that would physically tear into lps coral to get food out on occasion.

The other thing is that poly bail out isn't good, but also isn't necessarily a death sentence (although success rates after bail is not high, jts also not a 100% death scenario) If the polyp bails and seems to be intact and not melting etc, you can kind of isolate it. I've had a frogspawn do this and then I just put it in Tupperware with screen over the top for a while and it eventually did begin to create new skeleton that I could glue down. Lived for a few years after that till I broke down that old tank and passed it on.
