sponge filter. All 4 of my other fish were fine in the QT, same location. Except for the marine beta, which only had a problem accepting food. But he is doing fine in the DP.

Bump up salinity to 1.023. 1.021 is a little low and sponge filter IMO not enough oxygen and chemical filtration.

sorry to hear that mate.
Ive ran all my fowlr tanks at 1.022 all the time, it may be low for corals but not for fish, and a qt doesn't need any chemical filtration unless you are trying to remove medication, it should be a simple sponge filter, as long as there is surface agitation there will be enough oxygen. keep a couple of spare sponges in your main tank sump so they are ready for the qt anytime, once you have qt'd a fish, throw it away and use a fresh one from the sump and just keep that cycle going.
