For over a month. He had been very active. Eating great and even from my hand. I see no marks on him?!

What kind of butterfly? Any white spots or red streaks on his body? Can you post a photo?

I would get him into an acclimation box or QT for closer inspection. That doesn't look good.

Ok. I can set up a ten gallon fresh. I have never done a qt. I have read up on it. How often should I change the water. Do anything to the water?

Check this out for a quick "how to" on quarantine. How to Quarantine

You'll change the water any time ammonia is present.

I just took him off the intake tube...I though he was dead. No he isn't. He just tries to swim. But gets no we're and tumbles with the water flow. His eyes move around. But not a single more or anything on him

When I get him in the qt...do I give any meds? Or am I just watching him? What am I looking for?

I would just observe for right now (look for any unusual spots or marks); and also try to take some closeup photos.

Possible swim bladder injury?? You said he was trapped under a rock?
