So this fish has been in cupramine for four weeks now and this will not go away. It eats like a machine. Can it be ich or velvet? Or something else?

That is likely viral lesions. Sometimes this happens with tangs in copper.

How long has this been going on?

What is your copper level?

I put the fish into copper due to these spots. It has been. Going on 5-6 weeks. Copper is at 4.8 and I ramped it up. There is an orange shoulder tang in there that has shown no signs of this, yet. I am going to try fresh water dips.

So I did a fresh water dip and nothing fell off or changed. I’m leaning towards bacterial or fungal. Any medication recommendations?

You wouldn't be worrying about that fish anymore at a level of 4.8ppm cupramine.

It has to be bacterial or viral.

10 days of Nitrofuracin Green or spectrogram is where I would start. Feeding high quality vitamin soaked foods is a good idea as well.
