Help with Spiny Box Puffer

I have now had a spiny box puffer in QT for 45 days. I treated him at 2.5 with copper power for 21 days, moved him to observation and on day 12 showed Ick. Back into copper he went, brought it back up to 2.5. On day 10 and he still is showing ick.
I have successfully QT around 10 fish so far and have never seen ick on a fish with full therapeutic copper levels. I checked levels again today which I do every 72 hours and it was 2.46 on Hannah checker.

I have read that this fish can be tough, but what am I doing wrong. What can I do differently. I can’t risk all my other fish in my FOWLR and am starting to wonder if I should give up. Please help

Water quality is important during treatment. Pics under white lights will help also to determine if it’s ich or something different.
these fish are sensitive to copper. Hypo salinity is one safe method as is quick cure treatment.
My choice would be polyp Lab Medic for 10 days

Can you post a picture? Are the spots you are seeing on the body only, or also on the fins? Have you treated with prazi yet? All fish in this group have issues with parasites - like Typhoid Marys....back in the 60s we would avoid “bay fish” for being so diseased(grin).
