It's a bacterial infection the same things happen to my freshwater flowerhorns if the temps get low they call it hex in freshwater not sure in saltwater if it's the same
I think you could be right. I've had a look online and it looks exactly the same. Been to see my LFS and they agree so have started treatment. Hopefully it will do the trick. Thanks again.

Welcome to R2R! Heck of a first post and great response from everyone commenting. Noting this for future fish issues. Especially since have a puffer roaming the tank. Keep us posted if you can try capturing a pic or a video.
