Is this the beginning of brown jelly? Do I just frag this off?

I've had this hammer since it was a little frag for about 2 years, same spot every time. Noticed starting yesterday 1 head was more shriveled than the others, didn't think too much and now noticed today that one of the heads is like melting away and another is puckered in.

I don't have any medication on hand to deal with this but want to act fast. I am currently doing a 30% water change, do I just frag it off?

Euphyllia can get bacterial infections also unrelated to BJD. Ciprofloaxin works well on that also but if it is progressing rapidly then frag off dying heads to save the colony. Run big bag of carbon. If you see visible BJD then that is caused by unstable tank parameters causing stress on your corals which weakens their immune system allowing BJD to take over.

agreed, hard to see if the heads have died/are dying at this point in time. but if in doubt frag it and do a cipro treatment

I dose one dissolved 125mg pill in my 80g system 3 times over 6 days after lights out. So every other day for 6 days. Remove carb9n or chemical media. Leave skimmer on. Dissolved in tank water then pour contents into tank.

I noticed I only see 500mg pills, I'm assuming you just cut it into quarters?
