Long tentical anemone not looking good

So my purple long tentical anemone has always stayed put with his foot burried in the sand while also very responsive towards feeding. Today he decided to go on the move nothing out of the ordinary for anemones but he didn't stray to far. For about the past 6 hours he has been laying on his side with mouth gaping open not replanting his foot in the sand bed. I just noticed what almost looks like he has a hole in his food and his insides coming out of it. I am not sure but have never seen this can anyone might know what is going on here. If he continues to look worse I am going to pull him and place him in qt.
