Lymphocystis or Ich on my Royal Gramma?

IMO - the tail damage looks better, but the spots seem more numerous. I think it is ich. I think if you are going to try management, you’ll need to start right away. Try siphoning off the gravel tonight. If you are going to try peroxide, start now, plus use UV.
I can see 8 spots on the anal fin alone. kind of arbitrary, but fish with more than 30 spots total often don’t respond well to ich management.

Thanks Jay, any particular reason for siphoning at night? Could I do it at night and in the morning as well?

The ich tomonts (resting on the gravel/sand) release their swimming daughter cells a few hours before the lights come up. the idea is to try and siphon the tomonts out (too small to see really) before they do that, in the hopes of reducing the number of infective daughter cells that get released. It is NOT a sure fire process, you will no doubt miss many of the tomonts, but it is better than not doing that if you are trying to manage ich.
