Need help with next steps. Either QT or Fallow failed

Hi everyone, I need some suggestions on what to do next. Long story I'll try to make short... Ich broke out in tank, lost all but three fish. Moved fist out of DT and left fallow for 90 days. During fallow period, completed TTM on the fish and held them for observation until fallow period was over. Added them back to DT all groovy. Set up full QT system with UV and bought a White Tail Tang. Tang went through a round of Prazio Pro and 10 day treatment of Fromalin. He broke out with ich during the formalin phase but cleared up during treatment.

I moved him to the DT last night and boom white spots on fins this am. Gotta be ich right??

So I assume I screwed up a step throughout the process. So what do I do now?

Pull the Tang out and re-treat with copper.
Pull all fish out because its most likely the TTM failed. (ugh really)
Move to Ich management and by a UV.

I am so frustrated right now, please give me some hope and a plan

Are you sure it was Ich and not Lymphocystis ?

interesting, no I am not sure. Its just a few spots on his fins.

Update - its Ich

Is 10 days of formalin an effective treatment for ich? I would choose TTM, like your other fish, or copper.
