Pop eye ?????

I have a Rectangular Trigger who's eye have started to protrude from its head and cloud over. He's been in the Tank close to 2 years. The other fish appear fine. All still eating well. Any hope for saving him? Thanks.

Pop eye can be a symptom of another problem. The usual suspects are decline in water quality, specifically very high nitrates. Another common cause is physical injury and possible secondary bacterial infection. So I would test your water and make any needed corrections, if that is not the issue I would start the fish on Melafix. Look into what wide spectrum antibiotic everyone is using to treat fish with currently, remember it must be reef safe.

Had a recent bout with my Goldflake angel. Happened after my tank split a seam. Believe it was from a bump during the commotion. Both eyes. Great luck with Melafix and epsom salt. Took a few months, but she is back to normal.

I have a 75 gal set up with a 30 gal sump. Probably 75 gals of water total. Do 45 gal water change
each month. Nitrates have always been high. Tank been set up 2 years with 9 fish.
2. 4 in trigger
1. 3 in 3 spot damsel
2. Blue damsels
1. 3 in yellow tang
1. 1 royal gramma
1. Sgt Mgr
1. Flame Angels
Old Wet/Dry system. Crushed coral bed, good amount bleached coral, some tufa rock cover with red/brown Algie

Both eyes equally bugging out.

Do I treat the whole system or pull the one fish ?

Both eyes usually denotes a bacterial infection has set in. Perhaps it started out as an injury, but now that boo-boo has gotten infected. You can try the Melafix/Epsom salt route in your DT, as both are reef safe. However, if that approach doesn't work the fish could potentially be left blinded for life because you delayed real treatment. A more aggressive approach would be to QT the affected fish and begin antibiotic treatment on him. Erythromycin usually works pretty well on eye infections.

I'm not sure about the Melafix - follow the instructions on the bottle. But for Epsom salt, the dosage is 1 tablespoon per 5 gallons.

So I'd put 16 tablespoon in an 80 gal set up. What does it do?

Yes. Maybe mix it with some RODI & drip it into your sump using crimped airline hose, as it will raise your magnesium level. I think that's how I did last time in a reef tank. :D
