Premnas Gold Stripe - Help with identifying disease

Hi all,

We have a Premnas gold stripe in our tank.
Started Sunday Sep 25 not eating, swimming erratically and mostly towards the current of the recirculation flow. We suspected velvet because seemed she couldn’t breathe thus the swimming towards the current.

On Wed 28, she was eating very ocassionally, and discovered many small white powdery dots in her skin, watching at her through certain angles. Here are some illustrative pics of what i’m referring to, this kind of confirmed to us the velvet.

Also on Wed 28 we did the change of 10 percent water because we get hold of a QT and we needed to set it up.
On Thu 29 we administer PolyLab Medic into the reef. We noticed she regain appetite and wanted to eat more.

On Fri 30 we moved her into the QT, because she was in a very bad shape. the cooper finally arrived so to administer cooper into the QT.
This is a pic of her on Friday morning.

We started administering Cooper, apparently lift her up, she became better, her skin looked beter in general than in the previous picture, but the white spot concentrated in one place, now….

In general terms her attitude is better this morning (sunday) she was really looking for food and very responsive… so it kinds of throws away the velvet diagnosis, cause i don’t think (if it was velvet) she actually could get better so soon. Or can she?

Anyways, would appreciate help on diagnosing this…
The parameters of the tank are as follow both measurements taken while the Premnas was inside…

The polyp lab medic while it does help a small percentage of time is literally Peroxide salts ( same as giving peroxide bath) but you have what looks like Ich along with secondary bacteria. Problem here is which do you prioritze for treatment. In your case, suggested treatment which must be done in quarantine tank is to treat ich using coppersafe at therapuetic level 2.25 - 2.5 monitoring the level with a reliable copper test kit such as hanna brand, not Api
Also monitor ammonia level with a reliable test kit during treatment and would not be a bad idea to add an air stone for added oxygen during treatment. For bacterial issue, you can add Kanaplex by Seachem with coppersafe although I do not like to mix antibiotics with copper.
You will also need to leave display tank FishLess (Fallow) for at least 6-8 weeks to assure the cysts and trophants have completed their life cycle without a host and have died off

The spots seem too large to be velvet. I think it was an advanced ich infection. The new white area is likely a secondary bacterial infection from the ich infection. It might get better on its own, but if the spot is growing larger, you may need to treat with a broad spectrum gram negative antibiotic.
