My Rainbow Acan seems to be tearing itself apart is this normal or should I be concerned?

Nitrates 40 ppm (High and I'm working to bring them down)
Phosphates 0.25 ppm
Nitrates 40 ppm
Nitrite 0
Ph 8
Alk 10. 9 dkh
Mag (test kit on the way)
Calc (test kit on the way)
Salt: Redsea Coral Pro
Salinity: 1.026

IMM, that’s on the bad end of things.
It would bud from the base if growing.
From what I can see (and it’s blurry so forgive me)
It looks like receding tissue, maybe low nutrients
I might spot feed him every second day to assist in regeneration of tissue and ensure phosphate is stable and maybe 0.05-.1ppm as a reference.

It actually hasn't been this "fluffy" in awhile. I'm coming down from Nitrates around 200 ( almost nuked my tank by deep cleaning the sand bed).

Here's another pick, may be a little clearer.

If it’s just this one coral, in this one spot, nutrients would not be a factor.
It’s obviously damaged on the one side.
Could be anything, a fall, a sting, a crab….

Hopefully that’s a typo.
I was thinking more along the phosphate line.
Nope it was 200ppm and now it's 40ppm.

I did 15 Gallon water change by deep cleaning every inch of my sand bed. Nitrates went from undectable to 200 ppm.

It was my first time ever cleaning my sand bed like that.

I would recommend not doing that again. Not too seldom, it ends badly.
