Reoccurring Extreme ich/velvet blue tang in copper qt

I have this blue regal tang in qt with a tomini tang, 9 blue/green chromis, and a yellow coris wrasse. They have been in qt for 5 weeks with copper and I am not sure why the blue tang gets recurring extreme ich/velvet. Whenever nitrates get high in the tank then it gets what I think is either extreme ich or velvet. When I do a water change spots go away then come back like a few days later. All other fish are fine and the copper level is 2.4ppm copper power. Please help I am not sure what to do at this point.@Jay Hemdal

Im thinking skin irritation but best help you can offer is a couple of pics and even video under white light intensity - no blue to get clear ID on the fish's skin

Blue tangs often develop mucus plugs in response to stress. Once you post some pictures, I can get a better idea if that is the issue here or not.

He itches tho. Is this like a bacterial infection due to there being many bacterial blooms. It seems to get worse when there is bacterial blooms and it goes away if I do water change

Anything that causes stress can cause mucus plugs to show up: bacteria, ich, flukes and bacteria. Often they show up with no apparent cause.

Here is a video/photo, it shows the spots on its face and they are getting worse. Could you help me identify if this is a parasite or just mucus plugs

In the video, the fish is breathing too fast. The pale areas in front of the eyes is the start of head and Lateral Line Erosion. I can see some diffuse white spots around those areas, but I can't see clearly enough to say what that is.

The water is pretty turbid. I think you need to look into water quality issues first.....

I got a skimmer for the qt tank and did 80% water change yesterday. He has no more spots and no more sign of hlle or any disease. I also took the copper out of the system.

My tomini tang has always been extremely scared of people and I am not sure why. He swims with the other fish in qt and whenever it sees a person it darts into one of the tubes and breathes heavily. It does not come out to eat like the other fish so I just add some food then leave so that it will come out to eat. He is healthy and not sure why he does this. I am treating for flukes this week because I think some fish in qt may have flukes. If you could give me some feedback then that would be great!
