All other tank mates including 3 other tangs are fine. 175g mixed reef. Is this fungal? Thanks in advance for recommendations here.

This fish has a couple of things going on-
Bacterial which is likely related to a skin irritation as well as mucus cones often triggered by a skin irritation and may be mixed in with ich. Best recourse is to place in a separate treatment tank and treat with coppersafe at level 2.25 monitored with a Hanna brand copper test kit.
With this, often other occupants wont show signs but dont rule them out and keep close eye on them.
Additionally , monitor ammonia level with a reliable test kit and add aeration using an air stone. Treat with copper a full 30 days .
Not sure what foods you are giving the tangs but utilize foods such as LRS herbivore diet, plankton, mysis shrimp and Formula 2 frozen

Thanks for this, appreciate the prompt response. I am going to have a hard time setting up a quarantine tank in time, and very much doubt I can even get him out.

So definitely not fungal?

I already started treating with Rally broad spectrum. Treat for Ich too?

Benefits to using a UV sterilizer here? Or is that always recommended?

UV won't help in this case - those only kill free floating small single celled items like virus and bacteria. This skin issue is on the fish itself, so that would never get exposed to the UV.

Fungal diseases are very rare in marine aquariums, I've only seen a couple of confirmed cases myself. They also look different (more like grayish eroded areas).

I agree these look like mucus plugs. Trouble is, those can be caused by a number of stressors and may or may not involve bacteria. What's up with the white under shading on the belly? Has that been there all along, or is that a something that came along with the mucus plugs?

That undershading is recent too. He is also getting on a bit, not sure but I think 5+ years old, maybe as much as 10. Everyone else in the tank is fat and happy.

I can also see some minor fin damage to the dorsal fin and the tip of the caudal fin. That by itself isn't really a worry, but it could be contributing to chronic stress that in turn can produce mucus plugs. I'm not sure what to tell you about the pale coloration, except that it isn't normal.

Sorry I can't give you a specific cure for this....
