How long have you had the fish?
How long has it had this spot?
What other fish are in with it?

I know how tough it is to photograph fish, but the pics aren't super clear. Even with clear pics it isn't always possible to diagnose a disease visually....

It was QTd from a reputable source. I e had it for a month and just noticed it today. He is in a 180 with a yellow tang, coral beauty, 1 clown and 2 chromis.

I am having the exact same issue with my wrasse. I've had him for over 8 months and he showed me his/her mole about a month ago.

Based on what I can see, it looks like an injury. However, you are in a better position to tell - does it seem to be erupting from under the scales or is it on top of the scales? If the latter, then likely an injury. On other other hand, if it looks like it is pushing up from underneath the scales, that would point to an internal infection, and those are difficult to impossible to treat.
