Bought this guy about a year ago and it got lost in my tank. Found it stuck between some rocks about 3 months ago and it was almost dead. It has rebounded well over the last 3 months and I have no idea what it is. Can you guys take a stab at what this is? Kinda looks like a button scoly but its not... Maybe some kind of echinata?

I do believe you are right! Thanks for the diagnosis.

Should they be on the sand bed or in the rocks? Will it stay 1 polyp or multiply? Been looking around for these answers but not much info out there about these guys.

It should multiply heads, but very slowly. Pachyseptas are slow growers. It shouldn't matter where you put it as long as it's not in the top third of your tank.
 

Freshwater filter only or is it? Have you ever used an HOB filter on a saltwater tank?
