I dont know whats going on with my tourch but it's splitting from its skeleton in one spot I've never seen this before and I've been a reefer for almost two years so I'm the furthest thing from an expert

Have any of you seen anything like this before?

He looks unhappy in this image as I just moved him because my nem decided to relocate close by

It's dying. Probably got stung by the anemone. That tissue loss will likely slowly spread.

First off it’s not a torch it’s a wall hammer and yes it look’s like polyp bail out something is not right in your water chemistry please post your parameters

agree it looks like polyp bailout. The sand in the pic is very white, can you share more details the tank age, and water parameters?
