I think technically to be a pollutant something just has to enter the water that is harmful or poisonous.

From observation (going anecdotal here), that things like lotions and aerosols tend to be the most common. So make sure you don't put your hands/arms in your tank unless they're clean and don't spray anything near your tank that doesn't belong in the tank.

Freshwater filter only or is it? Have you ever used an HOB filter on a saltwater tank?
