What is wrong with my green Haitian anemone?

I have an Anemone inwater parameters are already premixed and done right I don't know if he's splitting or if he is sick and I'm posting here to find out I'm still new to this. I put him in Quarantine until I know more.

Looks like it got injured on the side somehow. Make sure the QT has enough nitrate, lighting, phosphate, and flow.
