Another worm: White with black head

Well, it's that time of the month when new things start popping up in your tank... Here's another I found today. It's very small - around 1/8"

Looks like a small ribbon worm.

I found a few in my tank as well but this is the only thing I've found on the internet as well... which is basically nothing.
