This is most commonly caused by a failed optical sensor inside the DDR. It can also be the result of liquid damage to the circuit board which is at the top of the DDR inside the triangular section below the two tube fittings.
Neptune replaced my optical sensors 3 times within the first year under warranty. Now that I'm out of warranty, I'm crossing my fingers that they don't fail again.
V1 sensors are garbage as suncrest has noted. Also as noted, over-filling the container kills the small PCB they have hidden at the top of the vessel (poor engineering tbh). If you have a multi-meter you can figure out where the problem is pretty quickly.
