No other issues. He's eating well, swimming around just fine and all of the other fish are fine with no symptoms. This primarily a 210 gallon fish tank with a few softees and some anemones. But the anemones have disappeared. Could he have eaten them and been stung in the process causing this.This isn’t really a symptom of a specific disease, how long has it been like this?

Thank you. I appreciate your supportI wouldn't worry about that then. I just Googled dogface puffer and saw a few pictures with similar coloration, it may not even be an issue at all.

As I understand it, Puffers can be that way. The can live with stuff for years and all of a sudden, decide one day that it's tasty. I know its a risk, but for some reason, my husband and I have never been successful at keeping a fish only tank with only rock and no Corals. It seems like the minute we put even just a few Corals in, we can get the tank stabilized.Moved our puffer to a different tank and he started eating all the soft coral
Exactly, our mostly fish and rock only has a few coral that nobody has touched in over a year lol have a porcupine puffer in that oneAs I understand it, Puffers can be that way.
