I think it may actually be a type of hair worm (trying to find scientific name right now), commonly misidentified as a spaghetti worm (though your type looks more like spaghetti than the ones carrying the name).
@EeyoreIsMySpiritAnimal knows more about this than I do!

Definitely a hair worm, a polychaete in the family Cirratulidae.
 

Freshwater filter only or is it? Have you ever used an HOB filter on a saltwater tank?
