It appears my Octospawn is struggling, any suggestions to help it?

I’ve had my Octospawn for 3-4 weeks. Initially it looked like this.

Now it looks like this.

Salinity is 1.024
Nitrate is 5 ppm
pH is 8.0
Phosphate is .29 ppm
Temp is 77.5 degrees

I have an Aquatop AIO 40 gallon. I’m using the protein skimmer that came with the tank.

How long has it been like this? This is what my torch looked like when it was about to bail out. What is your alk, mag, cal?
P.S, i doubt this is the problem, but most corals like 1.025-26, but i doubt this is what is the problem, just a little tip for your other coral

Yea I’m also guessing new tank. Salinity a bit low, nitrate a bit low, phosphate is high….. no alk test? the parameters seem unstable.
