Might have been some shock from a quick acclimation. Not sure from picture but looks like an ammonia burn on gills ?

Was he temp acclimated? How did you acclimate him exactly? Specifically, how long was he in the water he was shipped in after the bag was exposed to oxygen?

If it's ammonia toxicity, a 30-minute bath in MB might help, just need to be sure it's temp controlled and heavily aerated.

He was temp acclimated, I cut a small hole in his bag to check the salinity so I could match my qt to it, then taped the hole up while the bag floated in the tank

Spinal injury :( prognosis is not very good. Only thing you can do is epsom salt for swelling, but most eventually pass.

^^yup spinal injury, Epsom salts, feed well and just wait unfortunately 9/10 do not recover. He will eventually stop eating and just get blown around until he either passes or you euthanize him. Sorry :(

Frustrating, this is the second wrasse I’ve bought that the same thing happened too...I’m about to hang it up on wrasses, I have a starry blenny, striped blenny, oc clown, and a cardinal that have went thru qt just fine and been in my DT thriving for nearly a year, but wrasses just don’t work out...

I understand I’m 2/5 with Wrasses getting through QT. I had one suffer this exact injury and he lasted about 5 days before I euthanized him.
