Any idea what is causing this? Water levels are perfect and I’ve spent a lot of time watching to see if I have a nibbler.

Sorry it’s not the best photo. I hope between the photo and video there is enough for someone to be able to help

I also have a Duncan, Frogspawn and zoa that could not be happier. Every thing was great until 2 days ago when it receded and starting to lose its hammers. Now the second head is receding…

I also have a Duncan

is it normal for the one head to be stripped of it’s little “hammers”?
Is there multiple heads? It looks like one long head, also known as a wall hammer. If it's a wall hammer it's not good at all. If it's branching, not good either but not as bad as the wall hammer. To answer your question, no, it's not normal for the coral to recede like that. Unfortunately, your coral is unhappy. If other corals are doing fine, then we can rule out water quality issues. Is it getting the proper light and flow? Is there a possibility of coral warfare? Was it like this when you got it?

New angle.

It was very gently swaying in the water and happy as it could be and then Saturday afternoon it started to recede and today all the polyps are gone.

It Was 2 heads. The one pictured lower has lost all its hammers.

Was NOT like this when I got it.
No coral warfare. None of my corals are near each other.

Another video attempt
You have not recession but one side shows polyp bailout and can occur from low calcium, insufficient light and mainly too much flow especially if any recent changes were made.
additional cause would be high phosphate and low salinity - a few things to check
