Full disclosure, this is a freshwater tank I am having issues with fish dying in. The salt water tank is doing fine *knocks on wood*. As you can see the 3rd link someone recommended Furan 2 and General Cure along with levamisole.
The first two deaths were written off as "hey sometimes fish just die." I didn't get suspicious until this 3rd death.
I just dosed Furan 2 and General Cure today. I'm waiting on the levamisole to come in.

I know this is a saltwater forum, but this forum is very active compared to that freshwater forum. Hopefully posting here about freshwater fish is allowed.
I wanted to see if I'm doing the right thing? What's the white area on my fish? Maybe columnaris? Any other advice, tips, insight, comments, etc is appreciated.
I figured the freshwater fish would all be "a walk in the park" compared to the saltwater fish. Kind of ironic how we are struggling with the fresh water and not the saltwater.

This makes the 3rd dead fish.
First was an Albino Pleco on 1/12/19.

Next was an harlequin rasbora 2/19/19.

Now yesterday 2/24/19 I noticed another harlequin rasbora. Both were black. The remaining are mostly orange. Not sure if that matters.

Current parameters. I'm going to test nitrate again here in a bit but based on it never being more than 20 in the 2.5 months the tank has been setup I'd assume nitrate is not the issue.
Nitrate has always been 20 or below. I do 33% water changes, temperature matched with Seachem Prime added. I also run carbon and replace every 30 days.
Maybe this is a disease or aggression? I noticed one fish had white on top of it's fin. Is this a disease or normal?
I took a video of the fish. I tried to focus on the one with the white spot on the top of it's top fin.
Sorry for all the floating specs. The tank usually doesn't look like that. I picked up the fake plants looking for the missing fish. Stirred up some junk. I vacuum the gravel every water change.
