3 months is really early to add an anemone. But, you have, so lets try to get it healthy.
It needs more food from the environment. I unfortunately have little knowledge on LEDs. I believe Radions to be good. Make sure it is not too much light. Turn them down for a bit and gradually over two weeks bring back up to where you want them. Corals should be fine.
Clowns should not be and issue (they many times help an anemone adjust). Tang should not be an issue either.
Stir up the sandbed and blow off the rocks every other day. This will bring up some good stuff for the nem.
Do you have access to phyto? If so, try adding this to the routine daily.
I have never used cipro on a bta, but some have. I am worried as it is sooooo white.
I hope it pulls through for you. Good luck!

I dont believe bleaching is an indicator of a bacterial infection. Perhaps you could share your light settings and and someone familiar with them can vet them.

Also what size is this tank?

Mine did this as well, except it crawled under an overhang of a rock. Got down to the size of a #2 pencil eraser. As long as it stays attached let it be. It took almost 6 months for mine to recover. Have patience.
