Bad luck with whitetail tangs

Hello guys I just can't seem to keep a white tail long term. My 2nd attempt and in the QT the fish doesn't look well at all. I'm hopful it will pull through but I'm at my wits end. It's one of my favorite fish any help is greatly appreciated.

Both in Qt yes and both medicating in copper

Maybe try observation only for a week or two and see how things go before starting copper to make sure the fish is healthy first? I quit medicating and do observation only because I was randomly losing too many fish in QT and didn't know why.

Used bio media from 5 year old system like I always do for instant cycle. Was eating fine
Have you checked ammonia? I typically keep sponge filters in my sump for insta cycle also on stuff like this once used I toss them and get new ones. The added air to run them helps too. But even running seeded media I have had ammonia in qt while using copper. Just a thought.

I checked the QT tank has been running for 2 years with a yellow tail damsel in it lol.
