Bubble Tip Dying?

Hey all,

I have had my bubble tip for about a month now and he has been just fine. Been feeding him twice a week with shrimp. A week ago my wife cleaned the tank and since then he has been looking bad. Currently his tentacles have been fully retracted and he has a grey slime along them. Also the Nitrates have jumped to 20ppm when they have been running almost 0ppm. They are still 20ppm even after a 20% water change. The nem still shows signs of life but I am not sure if he's dying and if I should get him out of the tank?

Can you take a picture? How long has the tank been set-up for? What kind of lighting do you have? I would recommend another water change.
