It has three mouths. I'd say the larger polyp with two mouths is in the process of splitting and count it as "2 1/2" "heads". Others may not be as discriminating as myself and say it's either 3 or 2 depending their own standards.
 
I see 2. Middle is where they are coming together. Not common, but not rare.
