So I have had this duncan since December 10th. It came with two polyps which have been steadily growing in size. Finally last week i noticed a little polyp coming in at the bottom. This week it seems the polyp bailed as i found it floating around in the sand. Is it possible the other two polyps were blocking it’s light too much? Maybe it needs to be reglued in a different position?

Could be any number of things but likely too much flow. I think, in general, your PH, nitrates, and calcium are a tad too high while your alkalinity is too low. My Alk right now is 10.1 while the low range is usually considered 8 and max of 12.

It is not from too little light, the babies are always shaded by the full size polyps.

The mouths of the large polyps look extra inflated, not sure what's up with that. I don't believe the light is the issue though

Sigh. Your parameters aren’t going to cause an LPS to bail out if they are stable in those ranges.

Could be flow restriction? It almost looks like the coral is growing out of a hole in the rock?

Kind of, the rock is a bit cup shaped on top and the frag sits in the middle. One side may have been blocking the flow to it

It's interesting, in the past several years I have grown a couple of hundred heads of Duncan and they have always expanded from the main cluster - I have never seen or had them 'Budding off'. I'm wondering if something at night came by and plucked it off the side of the main group -- Like a crab.

I'd believe this more than anything. The parameters look good with just sort of high calcium. 7.2 alk is fine, I run mine lower. There are nutrients in the waters so it shouldn't be starving. Duncan can also take a ton of flow and are highly adjustable to flow and light.
