favia coral flesh ripping off

so i’ve had this favia coral in my substrate in a low flow area and i noticed it starting to rip away and i moved it again and it just sort of got worse and now it just looks completely dead at this point and i’m trying to wonder why

Its dying. Could be light, flow or pests. Can't tell from your first post what those parameters are, or how old the tank is?
