Does it move like a worm or is it stationary and does it seem aggressive or benign it may be more beneficial to your system than detrimental just keep an eye on it I have had vermetid worms and the population never took off I have also beat aptasia!

Too large and not spiraled enough to be a vermetid. I think it may be fan worm(feather duster) of some sort.
There are very large vermetid snails. Not just the small ones. In the video it is bringing back in a mucus net.

Also, There could be a large number of reasons for the hammer going down.

High alk, low alk, low magnesium. Low sg or high sg. The snail could be irritating the hammer with the mucus net. Too many factors to say what is killing the hammer.

Thanks every one for the input. In the video, you can see the worm or what ever it is, pulling in the mucus. I took the video with my iphone, I don't know how to convert it to another file. My parameters have been good, I don't remember them off the top of my head, I'm at work right now. But I have three other hammers that are doing ok right now.

Should I try to get rid of the worm/snail in the hopes of helping my hammer?

You can try, but also run water tests.

To remove the worm you'll want to use something like needle nose pliers. I don't like the super glue method. But you can dab some glue on the opening to starve him to death.
