Hammer coral doing weird

What happened to the head that is being weird?
It grew into 3 branch in a very short time
and now it looks like this sick form….
i dipped it in coral dip and it is still like this
some flat worm did come out last time i dipped it

How long have you had the coral? When did you dip it? Is that a night time picture?

Is that just white skeleton on the outside? Should have flesh down around the outside but looks like it is gone and only the newly exposed white skeleton is showing. If so this is not good.

What kind of flatworms came off? Have a picture? There are some that eat Euphyllia. And as noted by others. Post your parameters, how long you have had it, light and light settings etc.

A whole tank picture is helpful. Also listing tank history and length of establishment.
