Hammer doing badly for a month now has two circular holes in disk

These two little holes i noticed today. It dowsnt look anything like a mouth. Two perfect circle holes.. Is something eating it? You can kind of see the holes in the picture toward the upper part of the center.. Mycamera isnt great. My hammerhas looked like thia for over a month. All other corals are great. And this hammer for 6months was HUGE. In the last 2 weeks ive dipped once in reef dip and once in revive

That doesn't look good but I doubt anything is eating it. Though I guess it's possible. Anything change in your tank or this corals placement before it began looking miserable? It sounds and looks like it's on its way out, but let's hope for a recovery.
