Hammer looking sick

My hammer coral tentacles started to shrink on one side and the skeleton started showing. I just wanted to see what it could be. Should I soak it in coral revive? Will that even help? The other heads on my hammers are fine. No jelly showing up either. Parameters are fine. Any thoughts?

a list of parameters and test kits is important because your okay and my okay may be different. something that jumps out to me here is this coral looks bleached. do you know how much par its getting and if not what type of lights are you running on what settings?

Yup, need full list of parameters or we can all just take a guess.
Bleached out and really receded flesh.
What's the par also?
