Can someone please help id this hitchhiker that came on some live rock I just bought?

Looks like a really stressed kenya tree

Does this guy have a chance at making it? Or his he too far gone?

Kenya or Nepthea Coral or possibly Colt Coral. And yea, he's got a good chance of making it, those things are bullet proof.

Anything in particular I can do to get him back into good shape or is it just gonna take some time for it to get acclimated?

I'd just leave em be for awhile, he should bounce back. He's not entirely shriveled up, so he's already getting there.
