HELP? Zoas gradually all melting

Tank specs have been fairly stable. The past few weeks this issue seems to be increasingly getting worse. Trying to take pictures isnt the easiest. This is the next frag that it seems to be happening to, the top 2 polyps look like something stung them. Then they gradually close up and melt away. Any ideas would be highly appreciated.

The first thing I would do is a dip/bath in a Hydrogen peroxid dilution.

Please include the following: Lighting, age of tank and specific chem values.

The polyps appear bleached, as well as pinched.
