Is my toadstool dying??

I bought him a week ago and trying to get him to attach to this rock? It looks like he is loosing flesh, any ideas if he is dying and if I can save him?

It doesn't look healthy. Can you give some info on your lights and flow and parameters? They like medium light, medium-to-high flow, and some nutrients in the water.
 
They like water circulation, and they shed occasionally.

That coral will adhere to the egg crate as well as the rock. I would recommend removing the egg crate and wedge or rubber band the soft coral loosely but firmly in place and it will (may) grab hold of the rock if it is happy

Nitrate - 0.5 ppm
Alkalinity - 7
Calcium - 360 ppm

I have two SMATFARM 95 watt lights on for 8 hours, I have good flow in the tank as well. I tested these three today, what should I change and do you think these numbers are affecting this toadstool?

The nitrate is very low, too low. Do you know the phosphate number? I like nitrate at about 10-20, and phosphate at .05 to .1.

Soft coral does not rely much on alk and calcium, but your alk and calcium numbers also very low.

If you've had it in that same spot since you bought it, perhaps try moving it to a different location, sometimes a different angle of lighting or flow can help. Since the sandbed usually means lower light and lower flow, I would try moving it higher into more light and more flow. But keep an eye on the nutrients, add more fish or feed them more, but you need to improve it somehow.
