I have milli over 2 months and it pretty healthy until few days ago, the tip loat some tissue and polyps don't extend as it usued be. My parameter is
Salinity 1.025
Kh 7.9 ( constant)
Nitrate: 4-5 ( last week was below 2)
Phospate: o.01-0.02

Do you have calcium and magnesium measurements?

And what par is it getting?
Anything that could be taking nibbles?
 
My calcium is 450 and really stable. I sure with you there is nothing snipped my sps. Par at that area is around 200-250. I can see the bot of my mille still pretty healthy, but lot some tissue on top.
 
Did you recently do a heavy water change or change out carbon? Perhaps there is suddenly higher light penetration?
 
If the frag in the pic is 2 months old then it hasn’t been healthy. You would have seen encrusting growth at the base by now. It might be a sick frag? Did you check for pests? You might want to move it into more light and flow.

Should I take out and dip into coral RX then move it up to higher light ??

It's not all doom and gloom. I see some polyp extension. That's a good sign!
