Purple tang has one eye that is swollen and cloudy. I have had this tang for 23 years. No new fish introduced in years. Still eating and acting normal. Any suggestions on what it is or how to treat? The only thing i have tried different with the tank recently is to start adding probotics to help reduce phosphate.

May be a mechanical injury which is treatable with ethromyacin - Maracyn AKA Maracy 1, Not maracyn Oxy but it would be best to confirm before moving forward. Please post pic of fish's eye under white light intensity for confirmation

Update, I am assuming this was a wound. I did nothing and it's healed. So happy, i have had this tang 25 years.

I have an old clownfish that gets this every now and then. Every time time I think this might be the end for her, she heals up just like your tang did.

Wow, that's amazing! I would have thought that's the end of the eye. So glad it healed.
That had to be rough watching that for a week.

At time because its an injury , it will heal on its own and the maracyn will assure no infection
